Financial Aid forms for the 2017 - 2018 school year.

ALL FAMILIES must complete the on-line TADS financial aid information regardless of whether you receive financial assistance. This is required by the Daughters of Charity Foundation and Saint Patrick School.

Financial Assistance Information

ALL FAMILIES applying for financial assistance MUST complete the on-line FACTS form and the BASIC Fund.

2017 FACTS/PSAS Financial Aid Form - Due due to FACTS on February 15, 2017.

This form replaces PSAS that was used in previous years by the Diocese. This form must be submitted online. Be sure to include required documentation. This information is used by the Diocese of San Jose to award financial assistance. There will be a processing fee for this form.

Pastor's Signature Form - This is ONLY necessary if you are applying for a high school student.
If you are applying for an elementary student only, it is not necessary to obtain a Pastor’s signature

Deadline: Due by March 1, 2017 to Mrs. Constantino in School Office.

BASIC FUND for New students (Kindergarten – 8th Grade) need to complete the application form, Registration Agreement/ Release form, bring a copy of your 2016 Tax Form and if applicable *current statements of Food Stamps, Social Security, Unemployment/Disability, Retirement Benefits, Worker’s Comp, Foster Care benefits  and submit to Mrs. Constantino.
*Current means must not be older than 3 months
